Thank you for choosing Damascus Health Services for your patient's dependence on alcohol, prescription, or over-the-counter medications needs.
Please follow the link below to login to our eReferrals portal which will allow you to submit an online one page referral. Once your referral has been received, the team at Damascus Health Services will submit your referral to our team of specialists for assessment.
If your chosen Damascus Health Services doctor feels that your patient would benefit from treatment at Damascus Health Services, they will contact our Intake team and organise admission. We aim to make contact with you within 48 - 72 hours of receiving your patients referral. Our Intake Team will contact you after your doctor has been in touch.
Your account also allows you to view the status of your submitted referrals.
To assist you with this process, please have the following patient information at hand:
- Patient name and date of birth (DOB)
- Diagnosis details
- Referring doctor, requested admission/transfer date, requested assessment date
- Funding details (eg. Private Health Insurance, DVA etc.)
Important. If you are registering as a new user, you will also require a valid email address and contact phone number to submit referrals online.
